Lady Sumeragi is the grandmother of Subaru and Hokuto, and the twelfth head of the Sumeragi clan. Since the twins' parents passed away while they were still in their infancy, Lady Sumeragi was naturally the only family member left who could take on the task of raising them. She is an ancient matriarch who has garnered much respect, and this is reflected in the wise advice she imparts.

When Subaru was branded with the pentacle (the Sakurazukamori symbol) on his hands, Lady Sumeragi immediately sensed the danger that has dogged him. In order to keep Subaru from harm's way, she made him put on a pair of gloves without explaining why, except that he must wear them at all time. Though her efforts have failed to prevent the Sakurazukamori from catching up with Subaru several years later, they are testament to her acute awareness of the rivalry between the two onmyoujitsu clans, and the peril that Subaru is in if he were to come in contact with members of the Sakurazukamori clan.

Lady Sumeragi appears in the Tokyo Babylon manga and the X anime, sometimes giving Subaru directions on how he should handle evil spirits. Because she lives far away in Kyoto where the real Sumeragi residence is situated, she often communicates with Subaru by phone.
